How to Obtain your NeuroMove™:

1) Ask your physician to write a prescription for the NM900 and the duration you will need (typically 5-12 months). The physician who knows your condition should evaluate if this treatment would be of benefit to you. Your physician may also fill out the Medical Necessity form instead (or both).

2) Fill out the payment information (purchase or choice of lease plan). This can be printed from the "Prescription Forms" page. web-site. 

3) Send the Rx or Medical Necessity form, your choice of payment, PIF and a copy of your insurance card to us via mail or fax.  Our address and fax number are located at the bottom of this page.

4) When we receive the above information and your pre-payment we will ship the NeuroMove NM900. Click on "Lease Options" to order today.

5) When you receive the NeuroMove™ unit, a Patient Care Representative will call you to help you start the treatment.

6) A Patient Care Representative will then contact you from time to time to help monitor your treatment and solve any problems you may have.
